Initial Visit

We strive to give our patient’s with the highest quality of urologic care available and we want to ensure that your visit with us is as comfortable and pleasant as possible.

“ Please call us to schedule an appointment so we can better cater to your appointment needs and location requirements. Please have your insurance cards handy as we need certain information to help verify your benefits and eligibility. Providing this information to our staff will shorten your wait times. In the event we find any discrepancies with the information provided, we may contact you to get the corrected information. Failure to correct it may result in the cancellation of the visit.

To prevent delays and expedite your visit with us, we ask that you complete the following forms available for download or pick up at one of our office locations. Please bring these forms on or before the day of your appointment. Incomplete forms will be returned to you to fill out which will increase your wait time.

You will also need to bring the following items:

  • Your insurance card
  • A referral from your physician made out to Essex-Hudson Urology (if your insurance plan requires it)
  • A valid Photo identification (Valid state ID like a driver’s license, passport, etc)
  • List of allergies
  • List of current medications (prescription or over-the-counter) including name, dosage, frequency
  • Related X-rays, imaging, and Labs done
  • Records from the referring physician

You will also be expected to pay your co-payment or any deductibles at the time of the visit. We are happy to accept cash, check, visa, MasterCard, and discover. If you don’t have insurance, you will be required to pay in full at the time services are rendered.

You are requested to arrive 15 minutes prior to your scheduled appointment time. You will be required to leave a urine sample so drink plenty of fluids. You should plan on staying between 1 ½ to 2 hours. A thorough urologic exam will be performed which may or may not include an ultrasound. If you have to cancel your appointment, we ask that you kindly give us 24 hours’ notice to prevent a cancellation fee charge. We look forward to addressing all your urological care needs.

Follow up visits

Thank you for entrusting us with your urologic care. To ensure that you receive the highest quality of service and prevent delays we ask that you arrive at least 15 minutes before your appointment time. You will be asked to provide a urine sample so please drink lots of fluids before your appointment. You will also be asked about any updates or changes to your demographic information as well as any changes to your insurance information. It is due to this that we require you to bring your insurance cards during the time of your visit. You should plan on staying between 30 to 90 minutes which will depend on the circumstances of your visit. Again we kindly ask you give us 24 hours notice should there be a need to cancel your appointment.

Walk-ins and Emergency appointments Policy

In order for our physician to provide the highest possible medical care, we prioritize patients with a scheduled appointment. We do accept Emergency appointments and it is triaged depending on the severity of the medical condition. We also accept walk-in patients on a case to case basis only provided that there is availability in our physician’s schedule. Allot a minimum 3 hours for non-emergent conditions.
