A broad range of appointment hours throughout the day and evening is designed to provide you with the most convenient and efficient service.

In case of emergency
A highly competent urologist in our group is always available, 24 hours a day, 365 days a year, to treat emergency situations and to confer with patients and their families.

Initial Visit

On your initial visit, please bring the following items:

• Your insurance card
• A referral from your physician made out to Essex-Hudson Urology (if your insurance plan requires it)
• Personal identification
• List of allergies
• List of current medications (prescription or over-the-counter) including name, dosage, frequency
• Related X-rays
• Records from the referring physician

Payment and Insurance

We are pariticipating providers in most insurance plans. Please bring your insurance card and any necessary referrals or claim forms to your office visit. We appreicate satisfying all co-payments and the cost of treatments at the time of each visit. You may use VISA, MasterCard or a personal check if you wish. Our receptionists are experienced and can offer help with Medicare and other insurance procedures.

Medical Records

Please be assured that all patients' medical records are stricktly confidential. We will not give out any information to a third party without a patient's written consent. In the event that this situation aries, a release form will be provided for your authorization.

Languages Spoken

For the convenience and comfort of our Spanish and Portuguese patients, our staff speaks fluent Spanish and Portuguese. Several of our doctors speak these languages as well as Italian, Russian and Chinese Mandarin.
